ABSTRACT

Objective:To analyze the impact of the national tertiary public hospital performance assessment(hereinafter referred to as the national examination) on the functional orientation of the provincial public hospitals in Zhejiang province, for reference to improve the allocation of provincial medical resources and high-quality development of public hospitals. Methods:The data came from the hospital financial data of 17 provincial tertiary public hospitals in Zhejiang province from 2012 to 2021 and the DRG quality performance analysis report of the tertiary hospitals in Zhejiang province from 2020 to 2021. The information of the number of employees and actual number of beds at the end of the period, etc., were extracted to analyze the overall operation of the hospital. The ratio of outpatient times to discharged times and the proportion of discharged patients undergoing surgery, as well as the proportion of discharged patients′ day surgery and level-4 surgery in hospital and in province were took as evaluation indicators to analyze the functional positioning of tertiary public hospitals. Results:Compared with 2012 to 2018, the average number of employees at the end of the period and actual number of beds in 17 hospitals from 2019 to 2021 increased by 761 and 303 respectively, and the average number of hospitalization days decreased by 2.26 days. The average ratio of outpatient times to discharged times in the hospital decreased from 175.76 in 2012 to 67.51 in 2021. The average proportion of discharged patients undergoing surgery in general hospitals increased from 0.39 in 2012 to 0.46 in 2021, and that in non-general hospitals decreased from 0.67 to 0.43. The average proportion of discharged patients undergoing day surgery in hospital and in province increased from 0.20 and 0.03 in 2020 to 0.23 and 0.04 in 2021 respectively, and the average proportion of discharged patients undergoing level-4 surgery in province increased from 0.04 to 0.05.Conclusions:The national examination could be conducive to strengthening the functional orientation of public hospitals. After the national examination, the ratio of outpatient times to discharged times in the provincial tertiary public hospitals in Zhejiang province had declined as a whole, the proportion of discharged patients in general hospitals had increased, but the proportion of level-4 surgery in hospitals needs to be further improved. The author suggested that we should continue to strengthen the operation mechanism of functional positioning of provincial public hospitals, improve the service capacity of hospitals for difficult and critical diseases, and improve the performance evaluation system of tertiary public hospitals.

ABSTRACT

The article examines the influence of the Biologically Active Dietary Supplement (BADS) "Oleopren Hepa" as a hepatoprotective effect in the comprehensive treatment of acute alcoholic hepatitis. During the study, the authors obtained evidence of the effectiveness and functional orientation of the BADS. The materials for the research are the data obtained during the examination and treatment of 60 patients with acute alcoholic hepatitis. All examined patients were divided into 2 groups: 1st group (main), which received dietary supplements; the 2nd group (control) who did not receive this supplement. The average age of patients was 38.7 ± 9.3 years. The study was carried out based on the gastroenterological department of the city clinical hospital. As a result of the use of BADS in the comprehensive therapy of patients with acute alcoholic hepatitis, hepatomegaly was significantly reduced, and a decrease in the levels of total bilirubin, AsAt, AlAT, GGTP and cholesterol was observed. Additional intake of a specialized product enhanced the hepatoprotective effect of the main treatment. According to the tested biochemical parameters, the dynamics were better than in the control group. The use of the BADS, in addition to comprehensive therapy, increases the effectiveness of treatment of acute alcoholic hepatitis.

ABSTRACT

Background and Objectives: The long history of traditional medicine clearly demonstrates the role of nutrition in the prevention andtreatment of the disease. Of particular importance in modern scientific medicine is the simultaneous use natural biologically activecomplexes and mediccines. The objective of this study is to provide clinical evidence of the effectiveness and functional orientation of a newtype of biologically active additives (BAA) in the patients with vegetative vascular dystonia and chronic cerebrovascular disorder.Methodology: The principles of the modern science of nutrition on the role of individual macro-and micronutrients, and minor componentsof food in the correction of metabolic disorders form the methodological basis of the research. Results: The biologically active additive"Sophia" was introduced in the diet of the patients for 4 weeks at a dose of one tablet, in combination with 50 mg of Trental per day (the 1stmain group). The 2nd main group received an additional "Guta-Viva" additive. The 3rd (control) group received Trental only, at theprescribed dosage. The use of BAA had a positive effect on the relief of disease syndromes, and improved overall health. Conclusion: Theantioxidant effect of the specialized product, as well as the improvement of metabolic processes in the brain, was noted. A conclusion wasmade that the test product can be an effective remedy in the treatment of moderately grave vegetative-vascular dystonia. BAA is welltolerated by the patients and does not cause side effects.

ABSTRACT

Objective To explore the basic functional orientation of countywide healthcare alliances and how to maintain their stable operation and sustainable development direction. Methods 42 government officials, experts, employees of countywide healthcare alliances and residents were interviewed. The grounded theory was used to analyze data from the interview, with the core categories gradually extracted. Results Three basic functions were defined for such healthcare alliances, namely basic functions, stable operation and sustainable development. Conclusions The healthcare alliances prove an effective way to build the hierarchical medical system and improve people's health in general. Such alliances rely on good governance mechanism, reasonable balance mechanism of interests and scientific resource sharing mechanism.

ABSTRACT

Through the hierarchical diagnosis and treatment perspective to examine the functional status of public hospitals and hierarchical division of the status quo,it found that the current functional orientation of the problems and shortcomings. The impact on the medical order was analyzed.Functional ewolution of public hospitals should follow the hierarchical and differentiated development principles. All levels of hospitals needed to be repsitioned under the background of the hierarchy diagnosis.

ABSTRACT

Objective To explore the functional orientation of township hospitals in developed areas to achieve goals of the ongoing health reform. Methods Two rounds of Delphi consultation was made among 18 experts from around the country to learn the degree of importance of every function. Results The experts' average seniority of service in their fields was 23.28±10.41 years. The positivity coefficients of the two-round consultation were 100%.Kendall' S coefficients of the two-round consultation were 0.226 and 0.265 (p<0.01).These data are of statistical importance, suggesting consensus among these experts and the consultation meeting the needs of forecast. In these consultation, seven class-1 functions and and 27 class-2 functions were determined.Medical services and public health services account for similar proportion of work of township hospitals. Conclusion The functional orientation was feasible and forward looking, for reference of township hospitals in developed areas.

ABSTRACT

@# ObjectiveTo investigate the effects of functional point injection on upper limbs disorder in cerebral palsy infants. Methods120 cases were divided into 2 groups and each with 60 cases. The treatment group was given rehabilitation and functional point injection (injecting VitB1>, VitB12 in the acu-points selected at the disorder muscles) and the control group was given rehabilitation alone. All the cases were assessed with Peabody Developmental Motor Scale Fine Motor (PDMS-FM) and Modified Ashworth Scale (MAS). ResultsThe scores of both PDMS-FM and MAS improved in the treatment group compared with the control group (P<0.05). ConclusionFunctional point injection can improve the recovery of upper limbs disorder in cerebral palsy infants.
